Low Testosterone Therapy, LLC(Company) offers membership into an affiliate network of physicians, testing laboratories, and pharmacies focused around the management of Clients seeking affordable low testosterone medical therapies. The Client's tests are performed by third parties who also provide the results. Our affiliate doctors and medical staff order the initial pre-screen blood tests and review the test results. Should Client's pre-screen blood test results fall outside of medically predetermined acceptable ranges for testosterone therapy or should those results as determined by Company's licensed Medical Director indicate possible signs or symptoms of an unknown medical condition, Client will be referred to their primary physician for diagnosis and treatment. Company will not diagnose nor treat any medical condition that may become evident from the test results.

Company may collect Client's medical information to forward on to the affiliated network physician of Client's choice. All patient information will be protected under all HIPAA laws and regulations.

Low Testosterone Therapy, LLC and its website LowTestosterone.com are not internet pharmacies and do not diagnose any health issue or prescribe any treatment or prescribe any medications to Clients over the internet. Company's Medical Director and staff only order the pre-screen lab work that is required to determine candidacy of Clients for low testosterone medical therapy by licensed affiliate physicians in our network. Company does not dispense ship, distribute from, or store medications at our facilities or web sites or generate medication prescriptions. Any and all medication prescribed to Client is done so directly by the Client's chosen affiliate licensed physician from our network.

Company ONLY determines potential eligibility of Clients for low testosterone medical therapy to be administered by licensed physicians in our affiliate network, and is not involved in diagnosis or treatment of any related or non-related medical conditions of Client. Clients who are found to have submitted fraudulent information to Company for this program will be terminated from any health program offered by Low Testosterone Therapy, LLC or its affiliates. Any medication prescribed by one of our affiliate network physicians are for the sole use of the patient to whom it was prescribed and not to be transferred, distributed, modified, or used by any other party.

Confidentiality Statement

Your health is a serious personal matter and Low Testosterone Therapy, LLC and its affiliates understand that confidentiality is of utmost importance. Company uses only the highest level of customer and web site security features to guarantee your privacy and security. Refund Policy

There is no refund for testing once the Company's Medical Director has ordered the blood tests and Client has fulfilled the order.

Your Test Results

Company provides Client's lab results only to Client and Client's chosen affiliated network physician. Client may direct the Company in writing to forward Client's results to an additional 3rd party. Although some positive results such as HIV are required to be reported to certain government agencies only the minimum required information will be reported. Your privacy is important to us, and we use every possible measure to secure your privacy rights.

HIPAA: Health Insurance Portability and Accountability Act

This notice describes how medical information about you may be disclosed and how you can get access to this information. Please review this carefully.

In compliance with the 1996 Congressional act to protect the privacy of patients protected health information, Low Testosterone Therapy, LLC will safeguard all client information and will disclose or share only minimal information necessary for the following purposes:

Treatment: Information regarding current or past health information necessary for the Company and its affiliated network physicians to carry out appropriate care of the Clients which includes but is not limited to: History and physical, progress notes, laboratory reports, x-ray reports, operative reports, consultation reports, hospital discharge reports, hospital DNR , to be obtained from any clinic, hospital, skilled nursing facility, physician office or health care agency involved in Client's present and future care.

Payment: Information, if any, requested by an insurance company or intermediary necessary for the processing of claims for payment of services.

Operations: Review of medical records by any peer review organization, accrediting body, state or regulatory body for statistical or agency evaluation purposes only. Any information disclosed will be held in strict confidence and not used for any public disclosure.

If you feel that your privacy rights have been violated you may contact the Security Director of Low Testosterone Therapy, LLC. The Director will investigate all claims and provide you with the written report of their findings within 10 days. If you are not satisfied with the report and corrective action taken, the Director will provide you with an appropriate state or federal organization address and/or telephone numbers to file a complaint.

Company will maintain a log for each patient it services which will list what information was released and for what purpose. The patient has the right to review this log upon request.
